We have partnered with Vital Touch to provide Remote Patient Monitoring (RPM) and Chronic Care Management (CCM).

These programs are sponsored by Medicare and have proven to be very successful in helping patients stay on top of their healthcare needs via personal home-use devices and remote clinical staff.

Dear Patient,

CompleteCare would like to personally “Thank You!” for participating in our Remote Patient Monitoring program and as a bonus we are expanding that program to include Chronic Care Management. As as result of the success of this program, Medicare will now sponsor it and the expansion.

We have partnered with MedHab and Vital Touch to provide you with more tools and services to utilize and further manage your chronic condition. As you continue to use the medical device in your possession, our remote staff will work to support an agreeable simple action plan to address your overall health. You will enjoy this added service.

It is our goal to make your in-home care even better. Over the next several days, one of our remote staff will be in contact with you. We are especially excited about the chronic care plan that the medical assistant will discuss and develop with you. This will enable you to make greater strides with your overall health, which we hope will keep you out of the hospital.

Your responsibility, as a patient, is still the same and is to simply use the device daily and as directed by our staff. You must do this daily to capture the most up-to-date information regarding your overall health. It is also important that you continue to work with our staff as they are available to answer your

questions and concerns and celebrate your progress.
