CompleteCare Health Network (CCHN) is excited to inform you of two new health initiatives for 2024. We have partnered with MedHab and Vital Touch to provide Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). These programs are sponsored by Medicare and have proven to be very successful in helping patients stay on top of healthcare needs via personal home-use devices and remote medical staff.
These programs allow our remote staff to monitor you on a regular basis, allowing us to act more quickly when your health needs change. Devices include a blood pressure cuff, pulse oximeter, glucometer, calibrated weight scale or a therapeutic and fall detection device. These devices automatically send your vitals to a cloud-based system for review. You will have an assigned remote medical assistant who will work with you and serve as your personal medical assistant.
Your responsibility is to simply work with our assigned monitoring staff, and use the device daily as directed by your provider. Your consistency in using your device is important to capture the most up-to-date information regarding your overall health.
Complete the enrollment form. Upon completion, a remote care coordinator from Vital Touch will contact you within 24 hours to support your health needs.
